1.1. AA Praxis-H Szolgáltató és Tanácsadó Kft. (Registered office: 1165 Budapest, Nyílhegy utca 4 .; location: 1067 Budapest, Teréz körút 21. 1. floor 5./A .; company registration number: 01-09- 880726; tax number: 13936532- 2- 42, represented by: Levente Farkas, hereinafter referred to as the “Service Provider”) and the healthcare service established between the Client using the privately financed health care service provided by the Service Provider (hereinafter: the Client, the Service Provider and the Client together: “Parties”) (hereinafter: the Contract) shall be governed by these General Terms and Conditions (hereinafter: the “GTC”), unless otherwise agreed in writing by the Parties.
1.2. By using the private health care service, the Parties shall establish a legal relationship with each other for the provision of health care services for a fee. By using the service, the Client acknowledges that he / she has read, read, understood and acknowledged these GTC, and accepts the contents of the GTC as binding on him / her.
2.1. The Service Provider provides health and medical services to the Customer in compliance with the applicable legal regulations and medical professional protocols, as well as in accordance with the provisions of these GTC.
2.2. The Service Provider declares that it has the appropriate professional authority permits for the performance of the service, the necessary material and personal conditions, with regard to health care activities, and general professional liability insurance for its activities.
2.3. The Service Provider carries out its activities with the help of its employees and its contributors with a separate contract. The Customer acknowledges that the Service Provider is entitled to use an intermediary during the performance of the service, whose service is provided to the Customer as an intermediated service.
2.4. Locations for using the health service:

2.5. The Parties are obliged to cooperate in the use of the health service. Within the framework of fulfilling this obligation to cooperate, the Customer informs the Service Provider or its co-operating physician used by the Service Provider under a separate contract of all circumstances, facts (especially all previous illnesses, treatments, taking medicines or medicinal necessary to prepare an appropriate management plan and to carry out any interventions. The Client's obligation to provide information in connection with his / her own illness extends to all facts and circumstances (such as, in particular, infectious diseases, illnesses that preclude the performance of an occupation, condition) that may endanger the life or physical integrity of others. If the Customer fails to comply with the above information obligation, or only partially fulfills it, the Service Provider and its co-operating physician used by the Service Provider under a separate contract shall not be held professionally or financially liable for the consequences arising from the failure to do so.
2.6. The Customer declares that he / she uses the health care provided by the Service Provider of his / her own free will and with the provision of his / her right of free self-determination.

3.4. Service Provider reserves the right to change the above order order at any time without prior notice. In the event of a change in the order time, the fact must be published on its own relevant web pages (www.oktogonmedical.hu) immediately, but no later than within 5 working days after the change takes effect, and posted at the relevant order places, thus informing the Customer. If the Customer requests it in writing, the Service Provider is obliged to send the amendment to the Customer by separate mail.
3.5. The Service Provider is entitled to remuneration for the provision of the Service. The Customer is obliged to pay the fees to the Service Provider on time in accordance with the tariff list in force at any time.
4.1. Use of the Service requires prior registration on the part of the Customer, which the Customer can do in the following ways:

4.2. If possible, the Customer shall indicate at the reception of the relevant place of use of the Service at least 5 minutes before the pre-agreed time that he has arrived for the use of the Service. After logging in and recording the data, the Service Provider provides the requested Service to the Customer.
5.1. The Service provided by the Service Provider does not have social security financing (OEP), so the Customer can use it for a fee.

In addition to the above, the price list in force at any time has been posted at the Service Provider's ordering locations, and the Service Provider's customer service will provide information on the applicable fees at any time.
5.3. The Service Provider is entitled to unilaterally change its list prices at any time, simultaneously with their publication.
5.4. The Client acknowledges that in addition to the Service used by him, the treating physician may indicate the use of additional services. In this case, if the Customer also uses such ordered service, he is obliged to pay for this additional service. Prior to their performance, the Client may request information on the fees for the indicated additional services in accordance with Section 5.1. in the ways indicated in point.
5.4. The Customer may pay for the Service in the following ways:

7.1. The Service Provider makes every effort to serve the Customer correctly and without complaints. Should the Customer still have a complaint, he may make any complaint regarding the Service at the relevant customer service indicated in point 6.
7.2. The Service Provider accepts a complaint only from the Customer or his / her written representative, upon presentation of the invoice received when using the Service. If the relevant customer service is unable to verify the Customer's identity in a credible manner, it is entitled to request a written confirmation of the complaint or to reject the complaint.
7.3. The Parties shall settle any dispute amicably. If the related negotiations do not lead to a result, the Parties submit to the exclusive jurisdiction of the Arbitration Court operating next to the Hungarian Chamber of Commerce and Industry based in Budapest.
8.1. The Parties became aware of the data obtained during the Service in accordance with Act CXII of 2011 on the right to information self-determination and freedom of information. TV. XLVII of 1997 on the processing and protection of health and related personal data. in accordance with the law.
8.2. Parties are liable for any damages resulting from a breach of this data management and confidentiality obligation.
9.1. These GTC cover the Service Provider's private health services and govern in general terms in all matters that are not expressly provided for by the Customer and the Service Provider in an individual agreement.
9.2. The Service Provider is entitled to amend these GTC at any time. The amendment is published on the relevant website of the Service Provider:

10.1. Issues not regulated in these GTC are governed by Hungarian law and Hungarian legislation.
10.2. These GTC shall enter into force on 1 January 2018 and shall be valid until revoked or amended.
